Thank you for your participation in our educational activities. Now, please complete the attestation form at http://www.go2certificate.com to receive the contact hours you deserve.  This activity awards a maximum of 7.25 contact hours for supply chain professionals, healthcare executives and nurses and a maximum of 6.75 for pharmacists.

  1. Select the session(s) you attended by clicking the box to the left of the session title. Only one session may be selected in each concurrent time block.

If you select overlapping sessions you will receive a message to correct your selection. Please correct your selections and click the Next button again.
 
Your evaluation will load.
 
  1. Complete The Session Evaluations by using the NEXT button. Your input is invaluable.   At the end of the evaluations, please click the SUBMIT button.
 
  1. Please confirm how you would like your name to appear the certificate.  Include your professional credentials if you wish.

    Please select the box(es) requesting the accreditation(s) you wish to earn.  Multiple selections are acceptable.

  1. Confirm your hours. Click NEXT. 
A list will appear with the sessions you selected and the hours awarded for the accreditation(s) you wish to earn.
 
  1. Click VIEW button(s) to open and print your certificate(s).  Once you click the VIEW button you will also have the option to email your certificate to yourself. If you have selected multiple accreditations, please click the “Generate Bundle Certificate” button. Each of your certificates will be bundled into 1 PDF document.  In addition, your certificate(s) will be hosted on our website and can be accessed as many times as you like using the “Login” option on the main page.
    1. Pharmacists, you will not receive a certificate of participation. Your records will be automatically uploaded to CPE Monitor.

Continuing Education
 
Applications will be submitted for continuing education contact hours for healthcare executives (ACHE Qualified), Supply Chain Professionals (AHRMM), Pharmacists (ACPE), and Nurses (ANCC).
 
A maximum of 7.25 continuing professional education (CPE) credits have been approved by the Association for Healthcare Resource and Materials Management.

Healthcare Executives/ ACHE Qualified Education

It can be earned through educational programs conducted or sponsored by any organization qualified to provide education programming in healthcare management. Programs may be sponsored by ACHE, chapters, or other qualified sources, whether the programming is face-to-face or distance offerings (webinars, online seminars, self-study courses, etc.). You will receive a certificate of complete for 7.25 hours.
